FAQ

What should I bring to my appointment?

  • Driver’s license or State Identification

  • Insurance cards

  • Medication List

  • Medical records, x-rays, CT or MRI reports, etc., from prior doctor visits (if relevant to your visit with us)

  • Referral from Primary Care Physician, if needed

  • Payment for Deductible, co-pay or co-insurance

 

What other important things should I note?

  • Please have your referring medical caregiver fax your records to 404.296.9890 prior to your appointment (if relevant to your visit).

  • We invite you to review the services we provide. Under each section of our website, you will receive valuable information regarding conditions we treat, procedures we perform, infusion therapies offered, and our AAAHC certified endoscopy center.

What insurance and/or financial info should I be aware of? 

  • We accept many insurance plans offered in Georgia including Aetna, BCBS, Cigna, United Health Care, Medicare, Medicaid, Humana, Aetna, and Ambetter health insurance plans. 

  • If a referral is necessary, please provide the information when your appointment is scheduled.

  • Please call the office to check on your particular insurance plan.

  • Please also check with your provider to ensure there has been no changes in your provider panel.

  • We will file all insurance claims for you shortly after your visit.  

  • Deductible, Co-pay and coinsurance will be collected at time of service. 

  • All major credit cards and personal checks are accepted payment methods.

  • Atlanta Center for Gastroenterology, PC and Atlanta Endoscopy Center, LTD are separate entities. Patients who have a procedure will incur charges and receive separate billing statements from both entities. It is important that you verify your insurance is accepted by each individual entity by calling our billing department directly.
